The Russians do quite well in Olympic swimming, and if one notices, it is apparent that while some others are huffing and puffing through their mouths, many Russian athletes appear to practice exercises similar to Buteyko exercises before sports performances. Just as eating a bowl of spaghetti 15 minutes before an event would be detrimental, so is increasing the breathing. The Russian Natalia Molchanova has broken almost all categories of freediving records, and it is interesting to watch her breathing before a dive, as well as after a dive. Static Apnea training also includes minimizing breath.

To put it simply, Buteyko exercises are a set of tools, which allow asthmatics and others who are prone to hypocapnia (deficiency of carbon dioxide in the blood resulting from hyperventilation) caused by overbreathing, to return to stasis without (or with fewer) drugs, under medical supervision. Buteyko breathing exercises recalibrate the baroreceptors and parasympathetic nervous system, allowing for a lower breathing rate, and more normalized CO2, carbonate and pH levels in the blood, allowing the tissues to utilize oxygen better. Remember doctors using paper bags to stabilize asthmatics? (Do not breathe into a bag without medical supervision!)

Years ago I took a confined space training class, and we took turns blowing into an O2 detector after holding our breaths. Most trainees yielded 16-18% O2. I blew a 14% since I held my breath much longer. Assuming that the office had ~19% O2, this taught me that normal breathing is not very efficient, and perhaps has to be inefficient to allow for sufficient tensile strength of lung and vascular tissue, and delicate balance of blood chemicals. It occurred to me that more volume of air passing through the lungs does not necessarily mean that more oxygen is being passed to the bloodstream.

Two days after diving in the Pacific ocean, I was hiking in the snow up Cotopaxi at about 16,000′ (+4800m) above sea level. My extremely fit, but asthma-prone girlfriend began having shortness of breath. I also became light headed, and began experimenting with my breath as I trekked up the volcano. Several such times I inhaled deeply and compressed my chest with my intercostal (rib) muscles for a long moment. This seemed to help increase my consciousness even after I released the breath, and I realized that I had the power to change my physiologic state quite simply. It became clear to me how the mechanics of overbreathing/ heavy breathing and the partial pressure dynamics (elaborated by Buteyko doctors and scientists) may have much to do with the asthmatic response.

Many doctors now agree that overconsumption of nutrients (overeating) does not guarantee more nutrients to the tissues, but not many seem to consider respiration rate in the respiration equation, that overbreathing may play a key role in respiratory “disease.” In fact, some doctors no longer check respiration rate during an exam. It is commonly known that increased obesity results in metabolism dysfunction, large meals cause blood sugar spikes and metabolism accelerators (dieting drugs and supplements) due not increase stasis in tissues, making loss of fat more difficult in the long term. Similarly, overbreathing causes a spiraling of processes which decrease alvioli efficiency, disrupting breathing rate “calibration” and oxygenation of other tissues, decreasing health and oxygenation. Despite more oxygen entering the lungs, because of the low CO2 level, less oxygen actually makes it to the cells that need it. This overbreathing could be triggered by stress, anxiety, nasal congestion, allergies and other triggers and factors which cause people to breathe more frequently and deeply than their body would otherwise. One problem of low CO2 levels is a scientific hypothesis proposed by Artour Rakhimov, PhD, that chronic hyperventilation promotes disease and cancer. Previous research has suggested that chronic hyperventilation washes out CO2 from each cell of the human organism. Since CO2 is a dilator of small blood vessels, low CO2 concentrations lead to the constrictions of arterioles causing problems with blood and oxygen delivery.

In addition, low CO2 values cause inability of red blood cells to efficiently release whatever little oxygen they bring (the suppressed Bohr effect). The final outcome is hypoxia in the tissues, including the vital organs.

Some argue that “We are under constant stress because we don’t breathe enough,” without really measuring the actual volume of gases inhaled. Typically respiratory rate and volume increase when stress increases. Even the Johnson & Johnson Company sponsored research by University of Miami School of Medicine (Director Tiffany Field, Ph.D.) shows that massage increased pulmonary function in asthma patients. Once I was at the doctor, and a nurse assistant checked my pulse, bp and respiratory rate: the assistant reported that I had 18 breaths per minute, however I measured 6-7 breaths. I asked how she measured it, and she stated simply that “everyone has 18-20 breaths.” Such inaccuracy may lead to wrong conclusions by medical staff.

In 2005, the American Journal of Respiratory and Critical Care Medicine reported that “Hypocapnic but Not Metabolic Alkalosis Impairs Alveolar Fluid Reabsorption.” What this means is that a low CO2 level causes blood pH to change, and more fluid build up in the lungs. Thus the more a person breathes, the more CO2escapes, and more fluid may build up in the lungs. Buteyko theory includes the facts of the body’s attempts to regulate carbonates and alkalinity.

Although Buteyko Therapy originated in the Ukraine Forty years ago, and has gained medical acceptance in Russia, Australia, Asia and England, it remains a seldom used “alternative” treatment in the USA. My mentor, John Harris, brought the first Buteyko Therapy Practitioner to the USA in the 1980’s: Rosalba Courtney, ND, DO, DipAc, CA, an Australian osteopath, naturopath, California-certified acupuncturist, and chairperson of the Australasian Buteyko Association. The practice has been taught to many practitioners and countless patients who, under medical supervision, have overcome much or all of their dependence on asthma medications. John is said to breathe 3/min at rest, and 1/min during meditation.

Most meditation practices promote slowed breathing, and can accomplish these effects, when breathing is slowed more in ratio to the metabolic rate. Vocalizing the typical “oum” slowly while breathing through the nose only, will slow the breathing, and CO2 levels can stabilize toward a normal level. It is the respiratory rate and partial pressures that mainly control how much O2 in the lungs will pass into the blood. Proper singing and playing wind instruments may have similar effects, partly due to the increased partial pressure inside the lungs.

Most Pilates trainers, Yoga instructors and even physicians speak of shallow and deep breathing, but frequently don’t specify breathing rate, or encourage increased rate. Some yoga classes sound like an aerospace wind tunnel. However experienced yogis are rarely seen panting, and even with Ujjayi Pranayama, they breathe s-l-o-w-l-y, which along with the resistance of Ujjayi, causes partial pressure inside the lungs to absorb more of the oxygen while keeping enough carbon dioxide to maintain balance. Have you ever seen a Buddhist monk panting? Have you noticed that healthy older people do not breathe noticeably?

By slowing breathing to more fully utilize O2 efficiency and not depleting CO2 levels necessary for proper muscle and lung physiology, Buteyko breathing is said to enhance performance. Traditional Pilates and certain specific yoga practices that encourage faster breathing rates thus may cause an opposite effect. After I began to master these techniques, my breathing slowed during my workouts, and my efficiency increased. I learned that if my mouth would open during a run, it was time to back off, slow the exhale, until my efficiency increased again. Once you open the mouth, the breathing efficiency goes down the toilet. Now if a sprint is required, forget the rules, and when activity slows, then immediately slow the breath. Watch a video of Natalia Molchanova, world record freediver, and see how slowly she breathes after swimming under water for several minutes. I have also found that “catching” the breath only throws the balance off.

Since this also affects blood pressure, a US Patent has been issued for use to control hypertension. The American Lung Association has informed me that since most research has been in Russia and Australia, they are waiting for more USA trials on Buteyko before they comment or promote it for asthma.

One day I was examining the lungs of a cadaver and noticed that significantly diseased lungs with a fraction of their original available surface area still kept the person alive. Later that day as I was swimming (controlled breathing exercise, right?) I experimented with different breathing depths and rates, and thought of Buteyko’s theory, and that wide ranges of breathing patterns may have strong effects on what the lungs pass to and from the bloodstream, and resultant physiological responses. Apparently humans are overdesigned in many ways, thus improper function may occur for long periods before symptoms become apparent, and finally nudge a person to submit to crisis-control allopathy.

About ten months after NPR, and eight months after Oprah Winfrey started a neti bowl craze, I noticed that neti bowls were more popular on the net, but at a drugstore I asked two pharmacists for “nasal irrigation such as neti bowl or neti pot” and they did not know what it was. Rarely is time on the side of the operating room. In many cases, every minute costs dearly. And more and more often, pressure ulcers are being seen as a potentially expensive outcome of time spent in the OR, one that can create ripple effects throughout the entire healthcare system. That pressure sore that’s found on the nursing home resident may very well have started in the OR.

“Awareness of OR-acquired pressure sores has increased over the last few years,” said Dan Allen, a consultant for STERIS Corporation, Mentor, OH. “Longer surgeries are being performed on higher risk patients. The risks of developing a pressure ulcer as the result of an OR experience is increasing every year.”

“Time is one of the biggest issues in pressure ulcer development,” explained Allen, who specializes in surgical table accessories, patient posturing, pressure management and pressure sore prevention. The developer of the Allen Stirrup, he has spent the last 25 years devoted to developing posturing devices that protect patients from injuries that may occur as a result of lengthy surgical procedures.

The Association of periOperative Registered Nurses (AORN) 2006 Recommended Practices for Positioning the Patient in the Perioperative Setting, state that “procedures longer than two and one-half to three hours significantly increase the patient’s risk for pressure ulcer formation.”

While AORN guidelines recommend pressure relief surfaces for surgeries lasting longer than two hours, “pressure sores can start to form in as few as 20 minutes,” acknowledged Michael Bredal, vice president, sales for Action Products Inc., Hagerstown, MD. “It’s important to think about how quickly they can form.”

Complicating the matter, said Allen, “When a patient’s under anesthesia, they can’t tell you what hurts. The highest level of risk occurs when the patient is least able to provide feedback to deal with it.”

Therefore, “Vigilance and making sure they’re using products that are appropriate is key to pressure ulcer prevention”, said Bredal.

“Pressure sores develop from a combination of intrinsic and environmental causes,” said Allen. Patients are at greatest risk for pressure sore development as the result of pressures created at the bone-tissue interface. Bony prominences (heels, sacrum, shoulder blades, back of the head and elbows) are the most susceptible sites. Allen calls these sites “the landing gear” because they are the primary support structures interfacing with the table surface. “The goal of posturing a patient to achieve pressure relief is accomplished when pressure is redistributed away from the bone-tissue interface to areas where the bone is no longer pressing into tissue,” he said.

Vertical and horizontal shear also contribute to the formation of pressure sores. Capillaries allow only one blood cell to pass through at a time. If you stretch or elongate the capillary enough, it can become so thin as to be occlusive. If that occurs, blood cells can’t get through and tissue can die. According to early studies, it takes only 32 mm/Hg interface pressure to close a capillary, Allen noted. AORN guidelines state “studies suggest that positioning devices should maintain normal capillary interface pressure of 32 mm/Hg or less.” (1)

If you’ve ever shopped for pressure management devices, or even a new mattress for your home, you’ve probably seen a pressure map. With areas that are considered to be high pressure marked in red, and low pressure areas marked in shades of white and blue, pressure maps are often used in an attempt to compare interface pressures to represent the efficacy of patient surfaces, but manufacturers cautioned against using them as the sole measure of a product’s pressure-relieving performance. Looks can be deceiving, they agreed.

For one, the scales representing pressure can be set for favorable results. Allen explained, “A number of comparative pressure maps utilize a scale of 0 (white) to 125 (red) to represent the quality of their pressure management surface. It would be better science for manufacturers to supply pressure mapping using a much lower scale range. Since interface pressures greater than 32 mm/Hg close capillaries, the pressure maps should be set at 0-50 with any interface pressure greater than 50mm/Hg being depicted in red. Unless lower scales are applied to this mapping, it will continue to be difficult to evaluate patient surfaces.”

Action’s Bredal pointed out another nuance related to pressure maps that buyers should be aware of. “The way that pressure maps are conducted is that you do an average of all of the sensors that are activated. There are pressure points that would go beyond 32 mm/ Hg, but on average they are below. That is the industry standard for measuring pressure.”

Added Michael Brown, senior programs manager for therapeutic surfaces, Kinetic Concepts Inc. (KCI), San Antonio, TX, “An individual pressure map really only represents how a surface performed for the patient being measured, by the technology (equipment) used to measure, and dependent on how the equipment was set up and calibrated. Bottom line, pressure maps can be misleading.”

As with your face, small daily assaults can result in persistent dryness, mottled texture, pigmentation spots and other evidence of early aging.

“Women are really taking advantage of anti-aging face treatments and seeing a difference, but then they neglect their hands, which can easily look a decade older than they should,” says Ava Shamban, M.D., a dermatologist in Santa Monica, Calif. The benefits of hand care go beyond personal TLC. “Your hands say so much about you,” says Karen Elizaga, president and founder of New York City-based Forward Options, which helps young women hone their interviewing skills for the workplace. “Well-kept hands show the world you care about the details–when they’re spotted and sagging, it’s the same as walking around in a wrinkled shirt.” That’s why we asked top experts for simple ways, below, to keep these oft-forgotten parts looking their best.

Shut out the elements

Whipping winds and dry indoor and outdoor air can strip moisture from skin, resulting in chapped hands and ragged cuticles. The solutions:

* Wear gloves to shut out winter air, and use a moisturizer that contains humectants, such as shea butter, glycerin and/or hyaluronic acid. These form a protective barrier on skin and cuticles to help them retain moisture. Tip: Apply while skin is damp to further lock in moisture. Best bets: SkinMilk Hand Cream ($5; at drug-stores); De~luxe Shea 25% Pure Shea Butter Hand Creme ($8;  for store locations), with glycerin; and Clarins Age-Control Hand Lotion SPF 15 ($27; ).

* Don’t forget your cuticles. “Cuticles serve to protect the nail matrix, where nail cells are formed, so it’s crucial to keep them hydrated and free of biting, tearing or picking,” Shamban says. Try Crabtree & Evelyn’s La Source Nail & Cuticle Therapy ($10; ) with moisture-rich panthenol.

Protect from chemicals

Exposing skin to household cleansers can rob it of moisture. (These cleansers contain detergents that wash away beneficial, natural skin oils.) Similarly, exposure to chemicals also dries and weakens nails and contributes to cracked cuticles, which can leave skin open to infection. Shamban recommends always wearing gloves (latex or rubber) while doing household chores. Her other tip:

* Apply hand cream before pulling on gloves. Try Vaseline Intensive Care Healthy Hand & Nail Lotion ($4) or Johnson’s Softcream Extra Care Healing Hand Cream ($4; both at drugstores). Your body heat will help the emollients penetrate while you work. But chores aside, a truly effective glove is one that moisturizes as well as protects hands; we love Bliss Glamour Gloves ($50; ), which contain nourishing jojoba oil and vitamin E (note: these are meant for pure pampering, not for household chores).

Slough for softness

Like the face, hands benefit from exfoliation. Getting rid of dead, dulling cells will make hands feel smoother and give them a youthful glow. But since skin on the backs of the hands has fewer oil glands and less underlying fat than other body parts, it’s less “tough,” so a gentle approach is required.

* Try a scrub that’s specifically formulated for your hands. * Pair chemical exfoliators like alpha-hydroxy acids (AHAs) with a lightening agent, such as kojic acid, licorice extract or vitamin C. Hair is the ultimate accessory; it can add to (or detract from) your overall look instantly. Keeping it in healthy condition is the most important thing you can do to help it look and feel fabulous. And, while it seems easy, this isn’t as simple as minimizing chemical treatments or slathering on a weekly deep conditioner. While these can make your strands softer and less split-end-prone, what really matters is the daily handling; this is what creates the most stress–and potential damage–to your tresses. To help your locks look their best, we asked top experts from around the country for their advice on how to baby your mane every single day. So whether your concern is dullness, damage, frizz or fragility, we have the answers to ease even the toughest hair-care woes. Read on for tips to achieve run-your-fingers-through-it hair.

1 Steer clear of plastic-bristle brushes. “The proper bristles are key,” says stylist Edward Tricomi of the Warren-Tricomi Salon in New York City. “A combination of natural boar bristles on either a round or flat brush are best for dry hair, while soft, rubber-toothed wide-paneled brushes are best for damp hair.” Our favorite brushes include the Mason Pearson Boar Bristle brush ($78.50; zitomer.com) and Aveda’s Wooden Paddle Brush ($17; aveda.com).

2 Brush before shampooing. A few gentle strokes on dry hair will help remove product buildup and scalp flakes, as well as stimulate the scalp and promote blood flow (which delivers nutrients like oxygen) to hair follicles. 3 Know your water. If your hair looks dull or is hard to style, the problem could be your tap water. According to Minneapolis-based Gordon Nelson, international creative director for Regis Salons, well water contains natural minerals (called “hard water”) that can leave hair lusterless and hard to manage and can impart a brassy, orange hue. Soft water, on the other hand, has fewer damaging minerals. (Ask your local water department if your water is soft or hard, or try using Robert Craig’s No More Bad Hair Days Kit, $20; robertcraig.com; with strips to test your water.) To rid hair of mineral buildup, suds up every week with a clarifying shampoo. We like Frederic Fekkai Apple Cider Clarifying Shampoo and Clean Conditioner ($18.50 each;).

4 Mist your ends with water before home coloring. The ends of your hair are more porous and, as a result, absorb more pigment. “Wet hair doesn’t absorb color as readily as dry hair,” explains Renee Patronik, a consulting colorist for L’Oreal in New York.

5 Trim your troubles. As the ends of your hair get older and damaged by rough handling, they become prone to splitting, Nelson says. Get regular trims, at least 1/2 inch every four to eight weeks. “Hair grows (on average) half an inch per month, so trim to maintain healthy ends,” says stylist Stephen Knoll of the Stephen Knoll Salon in New York.

6 Use color-protective products. Chemical treatments like color can damage hair because the chemicals have to penetrate the outer layer of the hair (or cuticle) to allow the hue to be absorbed, explains stylist Rodolfo Valentin of Rodolfo Valentin Atelier for Hair in New York. Color-protective products are specially designed to minimize dryness, keep color true and prevent damage. “They typically have more nourishing ingredients, strip less color and are less abusive,” Knoll explains. 7 Give wet hair extra TLC. It stretches and snaps more easily than dry hair does, so be extra-gentle with it. “Use a wide-tooth plastic comb while hair is wet; then, once it’s towel-dried, switch to a good brush,” says Jon Patrick, color director of the Mete Turkmen Hair Salon Plus in New York. And avoid wooden combs; wood can have microscopic divots that snag hairs. Instead try the Jilbere de Paris plastic shower comb ($1.49; sally beauty.com for store locations).

8 Deep condition once every two weeks. “These treatments penetrate the hair shaft and strengthen strands,” says Patrick, who adds that using heat (from a blow-dryer) can intensify deep conditioning, as the heat causes the cuticle to open and the ingredients to penetrate.

Attractive merchandising and constant promotion is vital to the success of private-label beauty care, according to experts.

“The drug retailers who have been successful [with private-label skin and bath] are the ones that corporately get behind the line,” said Valerie Stricker, vice president of sales for CBI Laboratories, which manufactures private label for Eckerd and American Drug Stores, among others. “They promote it. They advertise it. They take pride in it.”

“Drug stores develop brand loyalty through their private-label program,” said Colleen Booth, national account manager for CBI. “The consumer has to come back to them to buy that brand. At first, what draws them is the unique packaging and design.”

“It has to pop off the shelf,” said Stricker. “Drug stores need shelf-talkers and marketing collateral to educate the consumer. There’s not someone to do that in a drug store like there is in department stores and specialty stores. The packaging becomes important. It has to have a high-quality look and a nice design,” Stricker said.

High-quality private-label skin and bath programs attract shoppers in search of value and help distinguish drug stores from the competition.
